Blackbelt Collective LLC urges all members and guests to obtain a physical examination prior to attendance of any classes, activities or events. By signing below, you either agree that you have obtained a physical examination and are in good health and able to participate in all classes, activities or events or you have agreed to forgo such an examination and represent to Blackbelt Collective LLC that you are in good health and able to participate in all classes, activities or events. You further represent that you understand the inherent danger in participating in any class, activity or event, including severe bodily injury or death, and voluntarily assume all such danger arising out of or relating in any way to such activities. You further agree to forever waive, release and discharge any and all claims or causes of action of any kind that you may have against Blackbelt Collective LLC, Transmerica LLC, their owners, staff, instructors, employees, agents, volunteers, and heirs arising out of or relating in any way to such activities. Finally, by signing below you acknowledge that you have read the foregoing, understand it and agree to be bound by these terms.
Video/picture/audio disclosure and waiver:
Blackbelt Collective LLC reserves the right to use any photograph/video taken at any class/event sponsored by Blackbelt Collective LLC, without the expressed written permission of those included within the photograph/video. Blackbelt Collective LLC may use the photograph/video in publications or other media material produced, used or contracted by Blackbelt Collective LLC including but not limited to: social media, brochures, invitations, books, newspapers, magazines, television, websites, etc. I hereby release Blackbelt Collective LLC and its legal representatives for all claims and liability relating to said images or video. Furthermore, I grant permission to use my statements that were given during an interview or communication, with or without my name, for the purpose of advertising and publicity without restriction. I waive my right to any compensation.
